After discharge - 08/2009

This one-page flyer supplies blank spaces for clinicians to fill in and give to a patient at discharge. Included are spaces for doctor's name and procedure, activity restrictions, self-care instructions (incision and dressing care, pain medicine), when to call the doctor, and the time and date of the follow-up appointment. Space is also provided for the patient's information label.
